Microscopic endodontics

Endodontic treatment under magnification, i.e. with the use of a microscope or dental magnifier, is currently the most modern way of treating root canals. Root canal treatment occurs when the carious cavity exceeds the level of the tooth pulp. This is when significant pain is most often experienced. Excellent 3D diagnostics in the form of point-of-care tomography and microscopic visualization allow our specialists to accurately and tightly prepare and fill root systems. Microscopic treatment significantly reduces the risk of tooth extraction.

Repeat root canal treatment

Repeat root canal treatment is performed when its original form has failed, that is, the tooth canals have not been completely filled. In such a situation, a common complication is peri-root lesions (cysts,) which cause pain or exclude the tooth from prosthetic treatment.

Repeat endodontic treatment under the microscope is a more difficult procedure than primary treatment, because the doctor must additionally get rid of the old filling and reshape the original canals. The rest of the procedures are similar to primary treatment.

Course of primary root canal treatment

We speak of primary root canal treatment when the pulp of the tooth becomes infected with carious bacteria and the need arises to clean deep carious cavities. The first step is to anesthetize the tooth. The next is to seal it tightly by placing a cofferdam so that the fluids used to rinse the canals do not irritate the surrounding tissues. This is followed by finding the canal outlets, the number of which depends on the number of canals. Our next task is to work out the canal with precise hand and machine root canal instruments, removing inflamed tissue. In the meantime, we rinse the dental canals profusely with fluids that dissolve necrotic tissue causing inflammation. Once the canals are prepared, they are filled with a thermoplastic material. It is worth noting that at each of our chairs we have the possibility to carry out radiological verification of root canal treatment, which significantly increases the standard of treatment and gives a guarantee of its correctness.

Does root canal treatment hurt?

Microscopic treatment in our office is always performed with local anesthesia, making it a painless procedure. Unfortunately, pain occurs when the patient decides not to undergo root canal treatment or tooth extraction. Then the growing inflammation of the pulp causes necrotic tissues to penetrate the bone, which is the cause of severe pain. Therefore, in the case of pulpitis, a quick decision is essential.

Endodontic microsurgery

The use of endodontic microsurgery methods is an additional option used when repeat root canal treatment fails. However, due to the current high quality of primary and possibly repeat treatment, it is very rarely used.

The stages of such treatment include:

  • root resection, i.e. cutting off the pathologically altered root tip with retrograde root canal filling
  • Closure of perforations, i.e. restoration of damaged tooth tissue during root canal treatment
  • Removal of healing lesions such as, for example, cysts
  • Rhadectomy, which is the removal of one of the inflamed roots while leaving the other roots intact
